Live Inclusive

PA Inclusive believes people with disabilities should live where, with who, and in the way they want and that those who want to work can work in an area of their interest in a “typical” job in the community. Both work and living support services can and should be delivered to support you in the manner you choose. We offer a variety of Day Services and Supported Living, such as Companion Services, In-Home and Community Supports, Community Participation Supports, and Non-Agency Owned Supported Living Services through the ID Waiver system, all designed to help you build a unique set of services so you can live the life you want.

PA Inclusive currently serves Luzerne, Lackawanna, Wyoming, Carbon, Monroe, Pike, Berks, Montgomery, Bucks, Lehigh, Northampton, and Columbia counties.

Where someone lives is a very personal choice.

Our interest lies in helping support the person to live where and with who they want and with the supports they need to remain inclusive in their community. We also believe Inclusive Living requires long-range planning, so transitioning from living with family to a Supported Living arrangement takes time and planning. We want to be there every step to ensure our clients are supported in the living arrangement of their choice. Supported Living can include living in your own home or apartment with or without a roommate or a Family Living environment.

Community Participation Supports offers a one-on-one staff to help involve you in the community in activities such as volunteering, learning to use public transportation, socializing in the community and in “Community Hubs’ (local businesses), and participating in activities that you choose.

In-Home and Community Supports (What used to be called Home and Community Habilitation) One-on-one staff who help you work on goals and help you at home and in the community.

Companion Services offers a one-on-one staff to be your companion for the day.

Supported Living Services are Non-agency owned living arrangements where you rent the apartment to remain living at home. You can live alone or with roommates in places like apartments, homes, and government housing. This service provides you with the amount of supports you need, from as little as 20 hours per week to 24-7 supports, with 24-7 on-call staff in case you need them.

Discovery Profiles and Supported Employment use a Discovery profile creates a baseline approach to employment and job supports and job coaches that help you transition and remain successful on the job.

PA Inclusive is a provider of services for people with disabilities who have a ‘Waiver for Service’ through the Pennsylvania Office of Developmental Programs (ODP). We provide In-Home and Community Supports, Companion Services, Supported Employment, and Supported Living Services.
